Alcaraz remains at world No 2 despite not having played competitively since April, though he signalled he was close to a return after posting videos of training sessions in the last few weeks.
Cincinnati Was a Key Preparation Stop
The Cincinnati Open is an ATP Masters 1000 event scheduled for August 13-23, and Alcaraz was set to return at the tournament he won last year after world number one Jannik Sinner retired from the final.
Cincinnati tournament director Bob Moran said: 'We know Carlos is doing everything he can to get back to playing tournaments as soon as possible.'
US Open Title Defence Under Pressure
The withdrawal casts doubt over Alcaraz's fitness for his US Open title defence, which begins on August 30. The Winston-Salem Open, which begins on August 23, would be the last chance for Alcaraz to get some matches under his belt before the US Open.
Alcaraz has yet to signal whether he intends to participate in the Winston-Salem Open, leaving his preparation for Flushing Meadows uncertain.
