Four months after the Bundibugyo Ebola outbreak in the Democratic Republic of the Congo was designated a continental emergency, an independent scientific review has urged authorities to maintain full response intensity while noting cautious progress in some areas.
Cautious Optimism Amid Uneven Progress
The Africa CDC Emergency Consultative Group, chaired by Prof. Salim Abdool Karim, completed an independent review of the Bundibugyo virus outbreak and expressed cautious optimism. Declines in cases and deaths in hotspots including Ituri are encouraging, but increases in some health zones, particularly in North Kivu, mean the outbreak has not yet reached its peak.
The Group warned against interpreting short-term declines as sustained interruption of transmission. Improvements in laboratory, treatment and community surveillance capacities have been noted, yet active transmission persists in parts of the affected area, requiring continued vigilance until gains are consolidated across all zones.
Persistent Gaps Threaten Response Effectiveness
The review identified critical gaps that undermine the response. Community deaths remain around 60% of reported deaths, and significant gaps persist in the identification and completeness of contact lists. The outbreak continues to disrupt essential health services, including immunisation, maternal and child health and antenatal care.
The ECG recommended urgent action to address these persistent concerns. Strengthening community engagement, early detection and trust-building interventions are needed to tackle the roughly 60% of deaths occurring outside health facilities, while contact tracing must be improved to ensure data quality and timeliness.
PHECS Status Maintained with Clear Next Steps
The Group recommended maintaining the Public Health Emergency of Continental Security (PHECS) designation and scaling up response intensity, especially in North Kivu. Africa CDC will work with the Government of the Democratic Republic of the Congo, affected communities, WHO and response partners to implement these recommendations.
Vaccination efforts should continue under approved protocols, with informed consent clearly communicating that the level of protection against Bundibugyo virus remains under investigation. The Panel emphasised that sustained interruption of transmission must be demonstrated before any de-escalation is considered.
